Please note that we are not a medical clinic and do not treat, diagnose, or cure any ailments. 

Revive Wellness Therapies requires all clients to complete a health history intake form that is reviewed and approved by our medical director prior to service.  Health history needs to be updated yearly or with any significant health changes.
